Log in to PRISM
Click on Self Service --> Student Center --> Enrollment Verification
Request Enrollment Verification
Select Processing Options
Choose from drop down — Allow to Print from My Browser or Request Institution to Mail
Academic Institution — defaults to UW Colleges
Check boxes you want to include:
- Include Program and Plan — will default
- Include My Earned Degree — will default
- Include My Term and Cum GPA
- Select desired term or leave blank for all terms — use drop down
Enter recipient Address Information
Number of copies required — enter number, defaults to 1
Send to my address — check box and Send To box will automatically enter your name
Address Type — use drop down menu, address will appear on next page, click ok to verify, address will appear in box
Edit address — click to make any changes
To enter additional addresses to this request:
- Click "Add" — an additional box will appear
- Sent to — enter Name
- Edit Address — click to enter address, click "OK"
- Address will appear in box
When all addresses are entered:
- Click "Submit"
- Request Enrollment Verification Report will appear
- Click "Print"
Note: Submit button is inactive. To use this feature, fill out form, print and bring to student services. - Print out form and bring it to Student Services Office
